LA Crockpot Chicken Cordon Bleu

3 whole chicken breasts, skinned and boned
6 sm. ham slices
6 sm. Swiss cheese slices
1/4 c. flour
1/4 c. grated cheese
1/2 tsp. sage
1/4 tsp. pepper
1 (10 oz.) can condensed cream of chicken soup

Cut each breast in half and pound until thin.
Place ham slice and cheese slice on each piece of chicken.
Roll up and secure with toothpicks.
Dip chicken rolls into mixture of flour, cheese, sage and pepper.
Chill chicken for 1 hour.
Place chicken into crockpot.
Pour soup over chicken in crockpot.
Cook on LOW for 5 to 6 hours.
Garnish with grated cheese.
